Your Yearly Health Checkup

Wellness visits are a yearly checkup with your primary care provider to assess and discuss your overall health and well-being. By seeing your provider annually for a health checkup, you can prevent health problems or catch them sooner. Your provider can also help you create a plan to reach your health goals and keep you on track.

During a wellness visit, your provider will review your health history, recommend screenings or tests based on your age and risk factors, and help you stay up to date with vaccinations. Wellness visits are also known as physical exams or preventive visits.

Stay Healthy by Reducing Your Health Risks

Once you are eligible for Medicare, you are also eligible for an annual Medicare wellness visit. This visit gives you the opportunity to meet with your provider and receive a personalized preventive care plan at no additional cost. Your provider will assess your health and medical history before developing a plan to help you reduce health risks.

Take advantage of this benefit covered once every 12 months by Medicare. When you call to schedule your visit, it’s important to ask to schedule a Medicare Annual Wellness Visit.

When you have a new or existing health concern, schedule an office visit with your primary care provider. Your provider will evaluate your symptoms and may order tests, prescribe medications or refer you to a specialist. This visit is intended to create a treatment plan for your new or ongoing health concerns to help you get well.

Virtual care connects you to a health care provider from your home, workplace or anywhere with an internet connection. Using My Sanford Chart, you can send a message to a provider by filling out a short, online questionnaire or see a provider face-to-face to discuss your symptoms, next steps and a treatment plan. Virtual care is a convenient visit option that may cost less than an office visit.
