Be Ready When the Whistle Blows

Complete your child’s sports physical at Sanford Health Windom Clinic – WAH Campus. Sports physicals are required to participate in middle or high school sports.

You have two options to complete a sports physical:

Traditional sports physical. Includes an examination by a provider and the completion of your sports physical form.
Comprehensive well child visit. Includes a sports physical exam, as well as a review and discussion of immunizations, family history, well-being and development. A well child visit is billed to insurance.

Please bring your child’s school sports physical form to your visit. The parent portion should be completed and signed.
